On the main page of the Medicare.gov website, click on Forms, Help & Resources on the right side of the toolbar at the top of the page. Click on Get Medicare Forms. The page lists situations in which specific forms are needed and is divided according to general forms, enrollment forms and appeal forms. Click on the situation that applies to you, open a link to the form you need, download the form and print it out. Some of the forms are available in Spanish as well as English.

On the main page of the Centers for Medicare and Medicaid Services website, click on Medicare on the left side of the toolbar at the top of the page. In the Medicare index, click on CMS Forms, and on the left side of the CMS Forms page, click on CMS Forms List. The comprehensive list has almost 200 entries as of February 2015 and has a search feature to locate forms according to form title or form number.
