For each of these forms, the provider must complete a detailed section about any visits the member has made to his office. In addition to his personal details, the policyholder must also include information for any members covered on the policy who are relevant to the claim. The Manulife website offers an online portal where policyholders can manage their accounts, apply for additional benefits and file claims.
