A teacher observation form collects information about how the teacher has set up the classroom or learning environment and how she teaches the lesson, manages the class and assesses the students' level of learning. The teacher's knowledge and professional conduct are also noted on the form.
The form may contain a checklist of items such as whether the instruction is engaging, whether there is a balance of teacher and student talk time, and whether the teacher paces the instruction properly. Some forms have an open format with a list of things to look for and space to note observed instances. For example, the observer might note whether a teacher effectively communicates goals and expectations to students.
