This begins with an email address, first and last name, home address, city, state, ZIP code and home phone number. The home address cannot be a P.O. box. The individuals may also provide his middle initials, suffix, and additional mobile and work phone numbers.

An applicant must then provide financial information, which includes annual income, residence status, and monthly mortgage or rent payment. Required security information includes the applicant's date of birth, Social Security number, photo ID number, and the name of the state in which the ID was issued. Acceptable forms of identification include a driver's license, state ID, passport, green card or military ID.

The application form on Citicards.com then asks whether an individual would like to authorize the enrollment of his account in the bank's paperless statements program. Finally, the applicant is asked to make several legal certifications. These include a confirmation that the applicant has agreed to receive the relevant terms and conditions, disclosures, and notice in electronic form, that the applicant has read and agrees to these items, and that the applicant accepts the use of his information by Sears.
