To find Indiana Medicaid providers, go to IndianaMedicaid.com, click on the Members tab and click Find a Provider at the top of the page. Find the phrase provider search in the second paragraph of the text, and click on it. Fill out all the information in the form, and click search. The website will present you with a list of Medicare providers.
For each Medicaid provider in the search results, the website includes the provider's name, full address, phone and speciality. There is also information about the provider type and program or delivery system. Patients can also view the provider's location on the map.
To search Medicare providers, users must choose the type of provider they are looking for, their speciality (it is possible to select several specialities at once), and whether the user wants to see only presumptive eligibility qualified providers in the search results. Users can also enter parts of the institution's or doctor's name and city, state, country, or ZIP code of where the provider is located.
Indiana Medicaid states that if you can't find your doctor in the search results, he might not participate in the Medicaid program anymore. The website has a list of terminated Medicaid providers.
