To fill out one of WSIB's PDF forms, visit WSIB.on.ca, and click on the Employers or the Workers tab depending on whether you wish to submit a form as an employer or a worker, as of 2016, notes WSIB Ontario. The Workers tab automatically loads a list of PDF forms, while the Employers page requires first clicking on the Forms link. Use the provided categories, such as "Claims" or "Report an injury/illness," to locate and download the form you need.

The forms are in a fillable format, allowing you to type directly into the text fields from a program such as Adobe Reader.
