With six different types of provider applications, it's important to make sure to select the correct one for services offered, CMS.gov advises. On a computer monitor, the Adobe Acrobat application lets you fill in each field and save the work as necessary. Another popular method includes printing out the application to type or hand write the answers into the fields. Provide answers for each requested field with accurate and detailed information.
After completing the entire application, double check each of the provided answers to ensure accuracy. At this time, print the form and save the final draft on the computer for your records. If you chose to complete a handwritten or typed form, make a copy of the final document. The completed application must contain an original signature, not a copied one, explains CMS.gov. When the application contains all the needed information and an original signature, mail it and all supporting documents to the Medicaid fee-for-service contractor for the particular state.
