Click the AT&T logo on the home page and click the File a Claim link on the File a Claim for Your AT&T Device webpage. Next, click the Let's Get Started link located in the center of the File a Claim webpage, instructs Asurion.

Fill out all fields in the Your Information section, advises Asurion. Required information includes the device owner's full name, mobile telephone number, contact number and email address. Click Continue.

Complete the What Happened section of the claim. All fields are required, including account information and a description of what happened, states Asurion. Click Continue.

Enter information about the device on the next screen, explains Asurion. Required information includes the make and model of the device. Enter the shipping address on the next screen, and then confirm all the information before submitting the claim. The deductible is billed on the claimant's AT&T account, and the replacement device is delivered within one to two business days once the claim is approved.
