To apply for Centennial Care, go to the New Mexico Human Services Department main page, select Looking for Assistance under What Are You Looking For, click on Apply for Benefits in the column on the left, scroll down, and click on Yes New Mexico under Apply on the Web, according to the New Mexico Human Services Department. Create a new account with a username and password, and complete the online application. When prompted, check that you are applying for Medicaid. Provide personal and contact information, a list of everyone who lives with you that is seeking health coverage, details on home income and expenses, and any other required information. Provide documents to support the information on the application, and then appear at a scheduled eligibility interview.

To access a paper application, visit the Apply for Benefits page, click on Medicaid Only under Download and Mail, and print out the form, the New Mexico Human Services Department states. Mail the completed form to the address provided on the form.
