A work order form allows a company to track requests for products or services from another entity, either a client, external partner or another section of the business. Sample forms contain the full structure and formatting of a completed form, but with placeholder text or blank areas that allow the person filing the order to customize it to the situation. The top of the form should include the name, address and phone number of the business completing the tasks, followed by a work order identification number unique to the project.
The form also needs to contain the name and address of the party requesting the work or goods with a separate shipping address if it is different from the billing address. The main section of the order needs to include ample space to write in the exact nature of the order, such as maintenance tasks on a building or the purchase of different products from a manufacturer. Each item also needs to include the cost, either for a single unit of each product or the estimated hourly rate for labor.
