To enroll in the Public Service Health Care Plan (PSHCP), fill out and submit an application form in print or online via Compensation Web Applications. According to the PSHCP website, you must also create an online member account and fill out the online Positive Enrollment form. At that point, you can print a benefits card or wait for it to arrive by mail.
The PSHCP application form requires you to provide basic personal information, including your name, contact information and past Royal Canadian Mounted Police experience, if applicable. You must choose whether to extend coverage to dependents, select a coverage level for hospital benefits and decide whether or not you want international coverage. If you were previously covered under the PSHCP, be prepared with the plan's identification number. As of December 2015, parents may add children under 21 to their PSHCP policies.
After your application has been accepted, you must visit the Sun Life Financial member registration website. According to the website, members may register through their benefits plan or retirement plan. The registration process requires basic personal details. If you have a PSHCP medical or dental plan, you need the policy number and your member identification number. For the PSHCP retirement plan, be prepared with your account number.
