To enroll in a Illinois Healthcare and Family Services health plan, create an online account, call and enroll by phone, or send an enrollment form to the Illinois Client Enrollment Services, explains the Illinois Department of Healthcare and Family Services. Each enrollment option requires choosing a health plan and choosing a doctor or clinic as a Primary Care Provider.
To enroll by phone, call the Illinois Client Enrollment Services' toll free-number anytime between 8 a.m. and 7 p.m. Monday through Friday or between 9 a.m. and 3 p.m. on Saturday, states the Illinois Department of Healthcare and Family Services. To enroll by mail, fill out the form sent to you via mail after calling the Illinois Client Enrollment Services, and send the form back with the preaddressed envelope that comes with the form.
If you enroll in the program and do not choose a health plan and a primary care provider, the Illinois Client Enrollment Services assigns one for you, notes the Illinois Department of Healthcare and Family Services. However, it is possible to change providers after enrolling in a health plan. You must enroll in a health plan if you have a Healthcare Family Services card, unless you have Medicare, live in a nursing home, have temporary medical benefits, are enrolled in the Illinois Breast and Cervical Cancer Program, or already have private insurance that covers both hospital and doctor's visits.
