Once you have selected the Forms & Pubs link from the IRS website, you are taken to a page that lists a few different tax forms. To find the W-9 form, select the link in the middle of the page labeled Find All Current Forms & Pubs. This brings you to a page that contains other tax documents and a search text box. You can type "W-9" in the search box to find the form, instructs the IRS.
The W-9 form appears as a hyperlink. When you click on the hyperlink, you are taken to the PDF version of the W-9. From there, you can fill in the form accordingly. The information you need to provide on a W-9 includes your name, address and social security number. You also need to sign and date the page.
