Health care providers complete the individual National Provider Identifier application through a secure, Web-based process or by paper application, according to the American Dental Association. Access to the online application is through the National Plan and Provider Enumeration System website. Individuals need to first create a login username through the Identity & Access Management System, advises NPPES. That username is used to access NPPES and the NPI application.
Before beginning the NPI application, providers should gather all necessary information and documentation because the application form cannot be saved to finish at a later time, cautions NPPES. The individual provider NPI application requires the applicant's name, Social Security number, date of birth, country and state of birth, and gender. The provider's mailing address, physical address and phone number of the practice location; provider type taxonomy; and contact person's name, phone number and email are also required. Some health care providers must enter their state license information.
When completing the online application, only the Next and Previous navigation buttons should be used, warns NPPES. Use of the browser's Back or Forward buttons could result in the loss of all information entered in the application.
Estimated completion time for the NPI application is 20 minutes, notes NPPES. Applicants who submit valid online applications should receive their NPI by email within one to five business days, notes the American Dental Association.
