The provider's mailing address, physical address and phone number of the practice location; provider type taxonomy; and contact person's name, phone number and email are also required. Some health care providers must enter their state license information.

When completing the online application, only the Next and Previous navigation buttons should be used, warns NPPES. Use of the browser's Back or Forward buttons could result in the loss of all information entered in the application.
