The Office of Environment, Health and Safety at the University of California, Berkeley, has made a template available for use at all departments at the university. In addition to sections closely paralleling the OSHA recommendations, this template contains forms for reporting unsafe conditions or hazards, documenting safety committee meetings, and assessing safety at office, laboratory, workshop and storage spaces, states UC Berkeley. Other templates are available for reporting hazard correction, investigating occupational accident, injury or illness, and recording safety training attendance. A department can download the template and fill out the blanks with its specific information.
