Applebee's customers can fill out a customer feedback survey by visiting MyApplebeesVisit.com or the specific Empathica link to the feedback form. Both links redirect to the same guest experience survey.
Customers must have their Applebee's receipt to complete the feedback survey, as the form requires a unique serial number found at the bottom of each receipt. MyApplebeesVisit.com also requires the total check amount, as well as the date and time of visit. The server name and check number are optional, though customers are encouraged to add this information when possible.
Though the feedback survey allows responses from more than 20 countries and territories, the survey itself is only available in English and Spanish.
