  1. Order a copy of your ChexSystems report

    Go to the CheckSystems website to order your report. ChexSystems requires you to have a copy of your report at the time of dispute. Give ChexSystems your information, including your Social Security number, driver's license number and address. Wait for the report to arrive by mail.

  2. Send a dispute to ChexSystems or ask the bank to remove the information

    Fill out a Request for Reinvestigation form, found on the ChexSystems website, and send it by mail or fax. As of 2014, the ChexSystems dispute address is Chex Systems, Inc., 7805 Hudson Road, Suite 100, Woodbury, MN 55125, and the fax number is 602-659-2197. Alternatively, you can ask the bank that reported the negative information to remove it.

  3. Submit a consumer statement

    Put an explanation on your report if you cannot get the account removed. This is your only recourse if neither the bank nor ChexSystems removes the account. Also, all negative information must be removed to have your name completely cleared from ChexSystems.
