OSHA requires employers to keep a log of work-related injuries, illnesses and deaths on OSHA Form 300, Log of Work-Related Injuries and Illnesses, notes the U.S. Department of Labor. The form includes fields for identifying the employee by case number, name and job title, and for describing the case, including fields for the date of injury or illness onset and information about the illness or injury. The log also provides a place to note if the injury/illness caused death, time away from work, job restrictions or other limitations, among other information.
