As of June 2015, individuals can complete the Medicare initial enrollment questionnaire at MyMedicare.gov, by phone at 855-798-2627, or at 855-797-2627 for TTY users, according to Medicare. The enrollment questionnaire is available online 90 days before an individual's Medicare coverage starts and for 180 days after the individual's entitlement date.
About three months before an individual's Medicare coverage begins, Medicare sends the individual a letter detailing how to complete the initial enrollment questionnaire online. The letter includes a user name and starting password, explains Medicare. The purpose of the questionnaire is to discover whether an individual has other insurance that may pay before Medicare pays, such as workers' compensation, an employer's group health plan or liability insurance.
An individual who is enrolled in Medicare Part A and wants to apply for Part B can obtain the Part B application in English or Spanish at Medicare.gov, notes Medicare.
