Under the Blue Advantage HMO plan, subscribers have access to primary care physicians who coordinate their care with standard medical care, prescriptions, and specialist referrals, according to BlueCross BlueShield of Illinois. Subscribers have the flexibility of choosing PCPs from a network of medical groups and independent practice associations.
As a part of the HMO plan, subscribers have no annual deductible or claims forms to fill out but pay a portion of expected costs as a copayment, explains BlueCross BlueShield. Benefits include regular physicals, immunizations, and screenings for common cancers as well as outpatient surgery and diagnostic testing as necessary. The plan also provides coverage for maternity care, mental health and substance abuse issues, and rehabilitation. The plan also provides coverage and discounted rates on other services, including emergency room visits, care while traveling, vision and hearing care, chiropractic medicine and wellness products.
