To qualify for the Affinity Health Plan, a prospective customer must reside in New York and meet the low-income guidelines, explains Affinity. As of October 2015, a single person must earn less than $16,105 per year and a family of four must earn $32,913 or less. A Child Health Plus program is available for families who do not qualify for Medicaid and whose children are uninsured. This program offers low premiums and access to a wide range of health care services.
