Affinity Health Plan offers free health insurance coverage to low-income residents of New York, states the company's website. It covers services such as primary care, dental work, eye care, emergency room visits and hospitalization. It also offers prescription drug coverage, family planning services and home health visits. For those over age 65, Affinity offers a variety of Medicare Advantage plans, including Plan D prescription drug coverage.
Members can receive behavioral health and substance abuse treatment through Beacon Health Strategies' network of providers and specialists, states Affinity. Vision care is available through Block Vision, a national vision benefits manager. CVS Caremark manages prescription coverage and allows Affinity members to use any pharmacy in the network, not just CVS. Members who are also on Medicare qualify for chiropractic treatment through Landmark Health Care.
To qualify for the Affinity Health Plan, a prospective customer must reside in New York and meet the low-income guidelines, explains Affinity. As of October 2015, a single person must earn less than $16,105 per year and a family of four must earn $32,913 or less. A Child Health Plus program is available for families who do not qualify for Medicaid and whose children are uninsured. This program offers low premiums and access to a wide range of health care services. The Medicare Advantage Plans are available for seniors at a variety of income levels.
