To apply, enter contact and personal information, preference for testing center, education, licenses and experience, clicking Save and Continue between each category. To add additional education or job experience, select the button at the top of the screen.

After inputting all of the relevant information, continue to the payment screen where applicants designate how they wish to submit supporting documents, whether by mail or via the online application. To submit supporting documents such as transcripts, diplomas and resumes via mail, print out the required cover sheet to include with each mailed paper. To submit online, select the option to upload documents and attach files, designating the type of file for each upload.

Pay the non-refundable processing fee by credit card, check or money order, or submit proof of fee exemption. Finally, submit the application.
