Apply for a Molina Healthcare plan by first choosing one of 11 states of residence from the drop-down menu at the top of MolinaHealthcare.com. Click on the type of plan you want on the left side of the Web page and follow enrollment instructions from that point, as of 2015.
For example, California residents have four options available from Molina Healthcare. Click on Medi-Cal, Medicare, Molina Dual Options Cal MediConnect Plan or Covered California to see plan specifics. Medi-Cal covers families with low incomes. Molina Medicare Options Plus plans are for patients dually eligible for Medicare and Medicaid. Cal MediConnect combines services from both Medi-Cal and Medicare, while Covered California provides government assistance for those who do not qualify for Medicaid or Medicare.
If you want the Medi-Cal plan, you must determine if you qualify from your county eligibility worker where you live, notes MolinaHealthcare.com. Print and fill out an enrollment form found on California's Health Care Options website, or call the Health Care Options help line at 1-800-430-4263. You may choose Molina Healthcare from a list of providers sent to you by the state. It may take up to 45 days to become a member of Molina Healthcare through the Medi-Cal enrollment option. If you have a new baby in the family, contact your county eligibility worker to add the infant to Medi-Cal.
