Apply for MO HealthNet at your county Family Support Division (FSD) office, outreach site or apply online at Missouri's DSS website. As of 2015, MO HealthNet Managed Care applicants can call or mail a completed Managed Care application to any MO HealthNet Managed Care Service Center.
Once determined eligible, you receive an approval letter that is valid for MO HealthNet services until you receive your identification card. Applicants eligible for MO HealthNet Managed Care must enroll in a health plan and show both the MO HealthNet and MO HealthNet Managed Care cards each time they seek medical services.
As of 2015, call the FSD Information Center to check on your application. Online applicants can check their status or receive support by calling the FSD Contact Center. On September 1, 2007, the Missouri Department of Social Services' Division of Medical Services was renamed the MO HealthNet Division.
