Click the Get Started link on the NYStateofHealth.NY.gov home page to begin the application process for the New York State Child Health Plus program, as indicated on the website. The New York Department of Health does not accept faxed, emailed or paper applications. As of 2015, call 800-698-4543 for additional information about applying for Child Health Plus.
Community enrollers are available to provide assistance with completing the application, collecting required documents and choosing a health plan, notes the New York State Department of Health.
A list of enrollers is available online at Health.NY.gov by clicking the Health Insurance Programs link under the Site Contents heading on the right of the home page. Select Child Health Plus, and then click Where Do I Go Apply link on the left side of the page. The list of enrollers include location names, telephone numbers and addresses for some locations. Near the bottom of the page is a list of Child Health Plus insurers separated by county.
Individuals who are certified disabled or blind must contact the local Department of Social Services to apply for Medicaid instead, advises the New York State Department of Health. The list of local Social Services offices is available on the Where Do I Go to Apply page.
