The Advantra Silver PPO plan has deductibles for some medical and hospital services apart from part D prescription drugs. The plan has yearly limits on out-of-pocket expenditure towards medical and hospital care. For instance, members within the coverage area in Adams and Allegheny counties in Pennsylvania can spend up to $6,700. These members can spend up to $10,000 if they receive services from any other provider out of the coverage area, explains Coventry Health Care. There is no limit on how much the provider can pay for services covered under this plan.
Members subscribed to the Advantra Silver PPO plan obtain diabetes supplies and services for 30 days, and any blood glucose monitors in excess of one per year. All members within Ohio and Pennsylvanian coverage areas copay $65 for emergency care services, but this service is free for members admitted to a hospital within 24 hours, says Coventry Health Care. For all coverage areas, members copay $50 for urgent care but pay nothing if admitted within 24 hours.
