A patient who has not visited a MUSC Health clinic since May 17, 2012 must request the activation code during his next clinic visit. After requesting a MyChart account, the patient receives an activation code that he uses to login and create a user name and password, explains MUSC. To activate the MyChart account, the patient visits the MyChart login page and selects Sign Up Now. He then enters the activation code, the last four digits of his Social Security number and date of birth. After clicking Next, he supplies the required information to complete enrollment.
MyChart is a free tool that permits MUSC Health patients to view their medical records and communicate with health providers online, says MUSC. Using MyChart, patients can see certain test results; send non-urgent messages to their doctor or nurse and receive a reply within one to three days; view past and future appointments; request prescription refills; and keep track of their health.
