Getting a tattoo typically hurts the most on the ribs, hips, feet, stomach and the area behind the ear. Other areas that are particularly painful when tattooed include the nipples, chest, ankles, and backs of the knees or inside of the elbow.
The less fat padding a body part, the more likely it is to experience extreme pain when tattooed. Any area where skin is stretched tight over a bone, such as the ankle, often needs multiple sessions for color to take properly. Anyone getting a first tattoo should consider a fattier body part, such as the upper arm, bottom or thigh, initially.
