"SOAP" is an acronym that helps nurses and other health care professionals remember the things they should ask and make note of while interviewing a patient. The letters stand for the words "subjective," "objective," "assessment" and "plan," in the order in which they are to be listed in the patient record.
The SOAP method was developed by Dr. Lawrence Weed for use by fellow physicians. Today, SOAP notes are the standard method of documenting and monitoring patient progress for doctors, nurses, therapists and emergency medical personnel.
