What Information Is Included in Lee Memorial Hospital's My Chart?

Lee Memorial Hospital's MyChart contains a summary of a patient's health and past doctor's visits, as well as a list of the medications the patient takes and most of his test results. Users cannot access test results of a "very sensitive nature" online, according to leememorial.org. A patient can also review, request or reschedule appointments through MyChart. He can also request prescription refills and send secure emails to his doctor's office.

MyChart only provides information on the individual user who has activated the account. Parents or guardians can request access to manage the health records and prescriptions of patients under age 18 by submitting a form and providing identification. Caregivers can also request access to manage another adult's records.

Users can access MyChart at MyChart.LeeMemorial.org. First-time users must request an activation code in person from any participating Lee Physician Group, Convenient Care or Florida Neurology Group provider. The user then can set up access to his account online. MyChart also offers a mobile app so that patients can access health records from their smartphones. The Lee Memorial Hospital website cautions that patients should not use MyChart to book appointments in cases of emergency, but instead should call local emergency care.
