Affinity's health plans all meet the New York State of Health's minimum essential benefits, reports Affinity. These include preventative and wellness services, maternity and newborn care, pediatric services, laboratory services and ambulatory patient services. Emergency services, hospitalization, rehabilitative services and devices, prescription drugs, and mental health and substance use disorder services are other mandated benefits. Plan holders can also take advantage of free preventive health services, such as mammography, prostate, diabetes and bone density screenings. Annual physicals for children and adults, as well as immunizations and vaccinations, are also available with no added out-of-pocket expense.
The AffinityAccess Platinum plan is for members who want low co-pays, deductibles and coinsurance when they require care, according to Affinity. Monthly premiums are higher than the other plans. However, the Platinum plan is specifically designed for members with significant or long-term health conditions. Members who want to opt for a lower premium can apply for the AffinityAccess Gold plan, though out-of-pocket costs are higher than with the Platinum plan.
