You should never create your own doctors note. That is called forgery. If you ever need a doctor's note for school, work, or some other occasion you should go to the doctor. If something is wrong with you to the point where you need to stay home your doctor will give you a note. If you miss work or school and forge your own doctor's note you could end up in worse trouble for forging the note.
