In legal terms, a formal sworn statement of fact is what is referred to as an "affidavit"." The word affidavit is medieval Latin for, "He has declared upon oath." An affidavit is signed by the affiant. The affiant is the person who is declaring said fact under oath. The affidavit is then witnessed by someone, formally and most often, the witness is a notary public.Affidavits normally have a
. header or a title, i.e., "Affidavit of John Q. Public" or "Contract Affidavit." The first paragraph of an affidavit normally states the affiant's civil information, i.e., full name, age, occupation, complete address of residence. For example, My name is [affiant's full name]. I am [age] years old, am working as a [job title], and currently reside at [complete residence address].Subsequent paragraphs constitute the affiant's core statement of facts, each paragraph thereafter is confined to a distinct affidavit fact, otherwise, called the affidavit's deed, i.e.[affidavit statement #1][affidavit statement #2][affidavit statement #3]The final paragraph is known as the Affidavit Footer or Affidavit Signature. An affidavit has to be signed by both the affiant and the person (or notary public), before whom it was sworn. The signature of the affiant is normally signed directly above the name of the signing person which is printed legibly directly beneath .An affidavit states the date it was sworn.A sample affidavit is as follows:Affidavit of John Q.
