A completed report on an official DA Form 2-1 maintains the Personnel Qualifications Record for a Non-Commissioned Officer (NCO) in the U.S. Department of the Army. Designed to help improve performance and develop highly professional NCOs, the form documents an NCO's record of service according to several different categories. They are: accountability and responsibility; training; leadership;
. physical fitness; and military abilities; competence; and values.Specific instances leading to updating the Personnel Qualification Record, according to Army regulations, include: reception and integration counseling; superior or substandard performance; crisis situations; referrals; promotion; and separation counseling. The section of the form that deals with responsibility and accountability comprises a review of the NCO's performance and sets goals relating to the officer's work ethic, efforts to take care of equipment and general sense of responsible behavior.An NCO is graded on leadership and military-specific abilities relating to the his/her self-development, commitment, loyalty, professional competence and influence on fellow officers. Further assessments focus on goals set by the NCO and his/her ability to participate in a continuing cycle of leadership development through institutional education, training, experience, feedback and reinforcement. Also, the NCO's special duty assignments are rated for advanced skills development and situations in which he/she operated beyond Primary Military Operational Specialties (PMOS) and unit missions.All NCOs are expected to stay physically fit, exhibit combat readiness and achieve optimal grades for having the stamina to complete assignments with speed, endurance and agility. In addition to the above evaluations, the NCO Personnel Qualification Record details dates and the NCO's duties. This is a critical part of the review process because it establishes clear communications between NCOs and commanding officers regarding specific expectations, PMOS title and Duty MOS code for actual duties performed. The commanding officer presents examples of excellence that the NCO has attained and that he/she has yet to achieve. The accomplishments of the NCO's subordinates are also considered. The form has sections for areas of special emphasis that include ratings on activities requiring priority effort, such as training and achievements for every stage of the NCO's qualifications process, preparation for deployment, artillery regiment and other assignments of importance during a rating period.
