Many employers choose to use some type of correction action form when disciplining their employees. These forms vary vastly in the type of information that they provide as well as contain within them. Most of the time the person who is filling out the form also has their name included within the form. Both people sign the form as a type of acknowledgement that they both agree about the terms that are written within the form. Plus the signatures also state that both people read over the document. These forms will have all of the information of the wrongdoing as well as a plan on how the employee should work on the problem or how they can fix the problem. Dates are always included, sometimes the date of the incident is also written within the form. There is also a date in which the problem should be resolved by. Each company has their own type of corrective action form and some companies even choose to use them when customers come in with a problem on their product. These forms help identify the issue and serve as a repair slip of sorts. These detailed reports include the customers service number, the problem they have perceived and then the repairer can then figure out what the problems are as well as an action plan on how to correct them.
