It is a sub-category of Microphilia. Use of the term in the United States comes primarily from the artist Toyogub, whose website has the following quote on its front page:

Literally meaning small lady, this interest is divided up into the categories of:

• Toy Size (Average about half a foot)
• Pocket Size (Small enough to fit in your hand)

The darker forms of Koonago artwork will depict such women being cut up or eaten provided they are toy size or smaller. One thing all Koonago artwork makes sure to depict is that clothing doesn't shrink with the woman in question.
